For care managers and clinicians
For Care Managers, Physician Offices and Senior Services
You see the signs first: missed medications, falls, weight loss, a spouse who can no longer cope. When a move to a licensed care home is the next step, Hamssa helps the family find one in the South Bay and Harbor area and stays with them through the tours, the paperwork and the move.
For professionals
How a referral works
Pass on Hamssa's number
Give the family her number, or call her yourself with the family's permission. She talks with them about needs, budget and area.
The physician's report
A California care home needs a recent physician's report before admission. Hamssa helps the family get the home's form to your office early.
Updates if you want them
With the family's permission, she tells you which home was chosen and when the move is planned.
Where Hamssa fits
- Patients and clients living alone whom you no longer consider safe at home.
- People with memory loss who need a home that meets California's dementia care rules.
- Spouses caring for a husband or wife who need help deciding what comes next.
- Families who speak Arabic or Spanish at home: Hamssa speaks both, and English.
“She personally met with me and took the time to visit multiple locations together so I could feel confident in making the right decision. Her patience, knowledge, and genuine care really stood out.”
“What impressed us most is that her support didn’t end once my father was placed. She has continued to check in, remains available when we need her, and genuinely cares about how he is doing.”
What the family gets
Hamssa refers only to licensed homes she has toured herself and checks each home's public licensing record for citations and complaints. She goes on the tours with the family, helps them compare, and checks in after the move. She works alone, so the family deals with her and not a call center.
Our family guides cover the questions you are often asked: the signs it may be time, what the physician's report asks, and when memory care is needed.
What Hamssa does not do
She does not give medical advice, provide care, or place in skilled nursing facilities. Her help is free for families; homes may pay her a referral fee after a move-in. See how placement is paid for.
Common questions
What does the physician's report involve for our office?
The home sends or names the state form it uses. The physician completes it, usually with a recent tuberculosis screening. Hamssa helps the family get the right form to you early.
Can we refer someone who is still living independently?
Yes. Hamssa also helps with independent living, and with board and care, assisted living and memory care when more help is needed.
What should we send through the form?
Only your own name, number and a general note. Please keep health information out of the web form; Hamssa calls you for the details.
Which areas does Hamssa cover?
Torrance, Redondo Beach, Rancho Palos Verdes, Rolling Hills Estates, San Pedro, Harbor City, Carson, Gardena and Long Beach.
