For professionals
For Social Workers and Discharge Planners
When a patient cannot return home and needs a licensed assisted living, board and care or memory care home in the South Bay, you can refer the family to Hamssa Benrekia directly. She works with the family and keeps you informed, at no cost to the patient, the family or your facility.
What to expect
How a referral works
You call, text or use the form
With the family’s permission, share a contact name and number, the expected discharge date, and the general level of care needed. Please do not send protected health information through the web form.
Hamssa contacts the family
She talks with them about needs, preferences and budget, suggests licensed homes that fit, and arranges and attends the tours.
She coordinates around the discharge
She keeps the family moving on the home’s admission paperwork and tells you where things stand, so the discharge plan and the move-in line up.
What Hamssa can and cannot place
- Licensed residential care facilities for the elderly (RCFEs): assisted living communities, board and care homes, and memory care.
- Residents on hospice, where the home is able to accept them.
- Short respite stays, where a home has an opening.
- Areas: Torrance, Redondo Beach, Rancho Palos Verdes, Rolling Hills Estates, San Pedro, Harbor City, Carson, Gardena and Long Beach.
Hamssa LLC is a placement agency for RCFEs. It does not place patients in skilled nursing facilities, and an RCFE cannot accept a person whose needs exceed its license. If you are unsure whether a patient is appropriate for an RCFE, call and talk it through. See hospital discharge placement for the family-facing version of this page.
Hamssa holds an RCFE Administrator Certificate and previously worked in palliative care, hospice and home health. About Hamssa
“We were connected to Hamssa through social services at Kei-Ai nursing facility. She contacted us immediately and soon after our conversation, we were given several choices for boarding homes, good choices!”
“She followed his case from the hospital, to the skilled nursing facility, and finally to the long term care facility. The whole time she would meet with his case workers, nurses, doctors and check in on my father.”
